A lot of proofs come in a promo pack. This was one of them. There have been several others.
Nightmares and Dreamscapes proof came with a jacket and a print of the house etc.
Hodder & Stoughton regularly release promo packs which have the proof in them. One of the better ones was The Green Mile 2 paperback.
There haven't been very many, if any, promo packs issued here in the US. Other than the ones you mention, I also own one for the UK Four Past Midnight that also contains a proof dust jacket, but not the proof.
Here in the US, publishers issue the proof and occasionally a letter from the publisher. Not much else.

I agree the US publisher's don't often issue much more than the letter and the proof, but they issue proof dustjackets quite often. I have sold quite a few recently.
As you know, the latest released proof dustjacket is for Under the Dome.
